Active for Life
Active for Life is a referral-based programme which is initiated with a referral from a relevant healthcare professional. Aimed at promoting habitual behaviour change, Active for Life is committed to helping individuals increase their health and wellbeing with the overall aim of improving quality of life.
What does the programme offer?
Active for Life takes a person centre approach to each individual who is referred into the programme. Our specialist Fitness Instructors will work closely with everyone referred to devise a physical activity plan which is tailored to their individual needs and goals.
With supported access to the gym, circuit based activity, swimming, and walking groups, Active for Life has an option for everyone irrespective of fitness level and ability.
Who is the programme for?
Active for Life is aimed at those who are currently inactive (or not meeting the recommended guidelines for benefit to health), live with one of more of the below noted health conditions, and require support to increase their physical activity levels.

How do I get involved
Due to the specialist nature of the programme, Active for Life requires a referral from a relevant healthcare professional. To get involved with the programme, discuss a referral with your GP, Practice Nurse, Nurse Specialist, or Physio.
