I understand that my professional contact details mentioned below and in the UNESCO Cultural Contacts Database’s Privacy Notice will be made public through the UNESCO Cultural Contacts Database.

 

I have read and understood information about the project in the UNESCO Cultural Contacts Database Privacy Notice. I understand that it is in my best interest to provide accurate and up to date details and notify the UNESCO Cultural Contacts Database if the details change.

 

I agree to take part in the UNESCO Cultural Contacts Database in the following capacity: I will provide my specific, professional contact details to UNESCO Cultural Contacts Database and following public domain information of my relevant cultural contacts outside Dundee – Name of the partner organisation (company name, organisation name or if public domain information – name of an artist), nature of the partner organisation, city and country where the partner organisation resides.

 

I agree that the data submitted by me will remain live on the UNESCO Cultural Contacts Database until such time as I request them removed or they are no longer accurate.

 

I understands that I can withdraw at any time without giving reasons and without penalty from the UNESCO Cultural Contacts Database. This can be done by emailing the withdrawal request to the email address of the responsible officer. Currently juraj.ciernik@leisureandculturedundee.com.
