How to get my Active Lifestyle Card

Membership applications can be processed at: Olympia; Lochee Swimming & Leisure Centre; DISC; Douglas Sports Centre, Menzieshill Community Hub, St Paul’s, Grove and Harris Swim & Sports Centres. Standard applications can also be processed via Join@Home

ACTIVE LIFESTYLE MEMBERSHIP TERMS AND CONDITIONS (Ts&Cs)

General Terms & Conditions

  1. Memberships are non-refundable. All memberships will terminate after 1 year
  2. Applicants must provide proof of address and photographic identity for all memberships
  3. Where family memberships are applied for, both adult applicants must be over 18 years of age and reside at the same address. Both applicants should be present at the time of application. Under 18s residing at the same address can be added to the family membership
  4. Anyone aged 17 and under can purchase a standalone membership. The parent/guardian for under 16s is required to provide emergency contact details and sign the Ts&Cs
  5. Refunds will not be issued for block booking activities e.g. swimming lessons, prior to taking out a membership. Memberships must be taken out prior to booking onto the block booking activities in order for the payment to be included in the membership
  6. The member is responsible for monitoring their own physical condition whilst utilising equipment. Should any unusual symptoms occur, the member must immediately advise an L&CD member of staff. Members should refrain from further usage and consult their GP

Leisure concessions are available to those who qualify under the following criteria (proof of eligibility is required):

  1. OAP’s 60+
  2. Unemployed and receiving Income Based Job Seekers Allowance/Employment and Support Allowance
  3. People receiving Income Support
  4. People receiving Council Tax Reduction/Housing Benefit
  5. People in receipt of Working Tax Credit (Subject to income)
  6. Support under part VI of the Immigration and Asylum Act 1999.

Membership Cards

  1. All lost, stolen and damaged membership cards must be reported as soon as possible to ensure a replacement card can be provided. If the membership is on a National Entitlement Card, please report it to Dundee City Council at Dundee House, North Lindsay Street to arrange a replacement.
  2. L&CD will not be responsible for the loss of any information stored on the card
  3. Membership cards must be produced whenever the member wishes to gain entry. Only the member may take advantage of the membership. Any attempted use, other than by the member, will result in cancellation of the membership with no refund.
  4. L&CD reserve the right to refuse any application for membership
  5. L&CD reserve the right to refuse admission or ask any member to leave if they are in breach of any of the rules. In such cases, memberships may be terminated and there will be no refund of any fees paid
  6. Activities and facilities are subject to availability. L&CD reserve the right to cancel, suspend, withdraw or amend any activity/venue. Venues may be subject to periods of closure for emergency, maintenance or other business reasons, details will be advertised prior to closure
  7. By taking out the membership, the member is confirming that they have read this agreement, understood its contents and has taken all such independent advise on it as they wish and accept all the Ts&Cs contained within it.
